Client Forms

To get started, please complete both forms.

  • The ‘New Client Form’ gives us all of the information to complete the death certificate.
  • The ‘Application for Cremation Authorisation’ allows us to book the cremation.

The applicant is you, the person authorising the cremation. This is usually the closest next of kin (spouse, adult child, parent or sibling) or the executor of the will. By signing as the applicant, you confirm you have the legal authority to make this decision.
